WHAT DOES CONCINNITY MEAN?

So glad you asked! Concinnity means the skillful and harmonious arrangement or fitting together of the different parts of something. Healing is the act of returning the body to its original state of concinnity.

WHAT IS A HYBRID DIRECT PRIMARY CARE?

Best of Both Worlds! ...A hybrid direct primary care (DPC) model is a healthcare approach that combines elements of both traditional fee-for-service primary care and the direct primary care model. Direct primary care is a membership model of healthcare where members pay a flat monthly fee and receive great benefits of traditional primary care without insurance. Included in the fee is unlimited access to care, same day or next day appointments, extended appointment times, discounted labs, prescription medicine filled while you wait and so much more. What makes us a "hybrid" DPC is we are also in network with most major insurance companies allowing to serve patients with and without insurance. Call for more information.

WHAT CAN I EXPECT AS A PATIENT?

All of our patients can expect shorter wait times, extended visits, and convenient direct access to your provider, who knows you and your family. No more rushed visits or waiting weeks for an appointment. You can contact your provider via phone, email or text whenever you need.

DO YOU ACCEPT INSURANCE?

Yes.  We are in network with most major insurance companies, however, because we are a hybrid direct primary care we are not able to offer membership to anyone who is insured by those we are in network with. Please call our office for more information.   For those with no insurance who need to see a doctor or provider, our services are provided on a membership/subscription basis.

IS MEMBERSHIP INSURANCE? DO I STILL NEED IT?

Your DPC membership is not an insurance plan.  At Concinnity Health we provide comprehensive care for chronic diseases, preventative health management and so much more to those looking for a family practice or for medical care even without insurance.   However, you may still need insurance to cover unexpected health care issues such as urgent care, emergency or some specialty visits.  There are some alternatives to traditional insurance companies that may actually be able to help you save money like joining a healthcare cost-sharing community. IS THIS THE SAME AS CONCIERGE MEDICINE?

There are similarities between concierge practices and direct primary care, but the two are different.  Concierge providers typically bill your insurance in addition to charging a monthly fee for their services which are typically around $200-$300 per month.  At Concinnity Health, we offer either membership only or insurance only option.  We do not combine the two.

WHAT IF I WANT TO CANCEL MY MEMBERSHIP?

We know life happens!  There are no contracts with your primary care membership and you are under no obligation at anytime to continue membership.  We do however, require a 30 day written notice of cancellation if you choose to no longer be a part of our practice. But why would you want to do that?  Please note that a $150 re-enrollment fee will apply if you wish to return. See membership agreement for further information or contact us directly.

DO YOU OFFER WALK-IN OR SAME-DAY PHYSICALS LIKE ATHLETIC PHYSICALS?

The great thing about our primary care is that you are guaranteed to get an appointment on the same day or next day, so it's almost like getting a walk-in physical or same-day physical, except there is no wait.  We offer sports physicals, employment physicals, college physicals, kindergarten physicals and we are able to complete your DOT (Department of Transportation) physicals as well.  

IS CONCINNITY HEALTH A FREE CLINIC?

We know that there are many individuals who have no health insurance but need to see a provider.  We are able to provide affordable primary care services to those with and without insurance or who do not have enough insurance coverage.  Call us to find out more!
